Indications for Testing

  • Concern for specific allergy (eg, hymenoptera) or progressive disease unresponsive to medical therapy (eg, sinusitis, asthma)

      • Screening test includes a balanced mixture of allergens most commonly associated with inhalant allergies

  • Skin prick or scratch testing – not recommended for patients with eczema

Differential Diagnosis

Clinical Background

Allergic rhinitis, asthma and anaphylaxis as well as sinopulmonary infections may result from inhalant allergens.  Allergen immunotherapy is established as an effective treatment for patients with IgE-mediated reactions to hymenoptera, allergic rhinitis and allergic bronchial asthma.

Epidemiology

  • Incidence – increasing incidence of both atopic and asthma disease over the last 4-5 decades
    • Incidence of rhinitis – 1/500
  • Age – onset usually in childhood
  • Sex – M:F, equal

Risk Factors

  • Genetics – family member with disease increases risk
  • Airborne irritants – smoking, pollution, pollen

Clinical Presentation

  • Sinus/nasal disease
    • Tender and swollen sinuses
    • Pressure in sinus area when leaning forward
    • Frontal headaches
    • Nasal obstruction and discharge
    • Post-nasal drip, rhinitis
    • Rhinorrhea
  • General malaise, sleep disturbances
  • Asthma symptoms
    • Wheeze
    • Cough
    • Dyspnea
  • Atopic dermatitis
    • Papular lesions on flexural surfaces
  • Ophthalmic
    • Conjunctivitis
    • Itchy eyes
